Documentation is poor. Author says that installation should be no problem for someone familiar with PHP, javascript, and CSS, and he means it. The documentation offers no insights in how to modify the most rudimentary elements of his code (like how to edit the phone number length requirement field). So, this is not the form an amateur wants to try to implement. But the worst experience, by far, is with support. It is utterly non-existent. He really should indicate that he does not offer support in the item description. Thats what authors do who dont want to be bothered. A complete waste of time.
